MN Diagnostic Assessment is now taking insurance through our parent company. All fees for rush orders are out of pocket costs.
Insurance companies we take include: BCBS MN, Medica, Optum, United Health Care, Cigna, and Sanford Health. We also take Medicaid through: HealthPartners, PrimeWest, and Blueplus.

Interview
Schedule an online or phone interview (typically 45-55 minutes).
Completed
Your assessment will be completed and emailed to who you've indicated within 5 business days. (Rush orders available)
